ss. at the top of an affidavit or acknowledgment is short for scilicet, to wit, and introduces the place where it was sworn.
Legal forms often open with a line such as State of Ohio, County of Wood, ss. The ss. stands for the Latin scilicet, to wit or namely, and states the venue where the oath or acknowledgment was taken.
It is not a person's initials or a page number. The place in front of it tells you which county officer took the statement and where the original may be filed.
