sacramentis provisus
Sacramentis provisus is Latin for provided with the sacraments; in a burial entry it means the dying person received the last rites.
Omnibus sacramentis provisus means provided with all the sacraments, that is, the last rites given to the dying. Munitus, fortified, is used the same way. For a woman the ending changes to provisa or munita.
Burial entries can also give the age, the spouse, the parents and whether the dead person left a will, so read the whole entry, not only the formula.
